Every Employee carries an identifier, a role, and a place in the hierarchy. Every action traces to a named Employee and, above it, to the person whose authority it acted under. There is no anonymous step.

An Employee works inside a tier. Anything above it is handed up, with the reasoning attached.
A record written as it happens
Decisions are recorded against the identity that made them, in an account that is added to and never edited.
